2010 Swim Suit Order Form

The logo suit remains the same as last season. Metro Swim Shop will be present during Registration for fitting and ordering. You will pay Metro Swim Shop directly for your order so please be prepared to bring an extra check for them if you plan on buying a new suit this season. The may also take major credit cards. Swim Suits purchased after May 21st, 2010 will not have the Team logo

2010 Summer Seasonal swimmers please fill out attached Insurance form and include $35.00 with your payment for Registration.

2010 Year Round Swimmers please bring proof of MD USA Insurance to Registration and no payment for insurance is required. (copy to be attached to registration appreciated)

Please write your main email on this insurance form. These forms stay with Coach Becky and are not disbursed to anyone else. This helps for quick and accurate registration.
